Performance Management Survey Comment Section

Originally from ticket #3193.

I think it would be nice to add a comment section after each section in the performance management survey  (relationship & Performance, core values, etc.). This would be optional. But I find myself after rating them that I would like to explain some of this, but there is no place to put it.
