N/A Option for Survey Evaluations

Originally from ticket #3426.

Please add a N/A ranking (now we only have Accountable, Work Quality, Team Orientation, Collaborative, Value-Add, High-Value) to the survey ranking selection.

We need a N/A criteria ranking to the survey so an overall average team performance can be properly reflected on the project summary.

There are times in a project when not all members work together so they are not able to evaluate the team member who they were not working with directly . The team is left with two choices, either complete the survey but with a too low or too high rating that will incorrectly be reflected on the member & project overall assessment or not complete the surveys at all as the average value of the surveys can not be correctly represented.
