Sponsor Evaluation on the Surveys

Originally from ticket #3426.

Please add the name of the sponsor to the survey, so the organization may have a complete team evaluation survey. Presently the team evaluation is incomplete as the Sponsor, who is a key team contributor, is not evaluated and may not benefit from feedback. Currently only the team members can evaluate themselves and the leader but not the sponsor.
