Add comments to each section of the individual performance review survey

Comment boxes should be added after each of the sections of the individual performance review survey:

• Relationship and Performance
• Player's Core Values
• Player's Basic Role

The comment boxes would be similar to the ones already used at the end of the survey. There would be a box to enter a short answer to a given question (questions to be determined), then another box to elaborate on that answer. Both the player and the coach would have a chance to fill in these comments. The player's comments would be visible to the coach when the coach is filling out the survey, and the coach's comments would be visible to the player when the survey is returned to the player for permanent record approval.
